This won't apply to everyone but its an issue that can occur that gives similar symptoms. Is anyone else able to reproduce this on their end? Why does Apple keep the message on the cloud even when I want to delete it? I have had this issue with each iOS 15 update since December 2021. Deleting messages syncs across my Devices however, toggling Messages o off/on in iCloud results in the deleted message being downloaded. All the messages on my phone uploaded to the Cloud and they also downloaded to my iPad. Yesterday I turned Message in the cloud back on. During that 30 day window I also deleted all messages off the iPad. I assumed there is an issue with my iCloud so I turned off iMessage on my iPhone and iPad, deleted the iMessage Data in iCloud and kept Message in the Cloud turned off for 30 days (Apple will delete all iMessage data in the cloud after 30 days after you disable it). If I toggle off Messages in iCloud and turn it back on, the deleted message returns. If I delete a message on my iPhone, it is removed from my iPad and vice versa.
